Please complete the Notification of Loss Form if you are a Colorado School of Mines employee and have experienced a loss involving Mines property. Once this form is submitted, the Office of Risk Management will submit the claim form and supporting documentation to the appropriate insurance carrier. For any questions about the form, please contact Mines Risk Manager Olivia Watson at olivia.watson@mines.edu 

For property loss involving Mines property, please fill out the Claims Spreadsheet and attach to your Notification of Loss submission.

NOTE:  All claims are subject to a $100,000 deductible. Contact the Office of Risk Management at olivia.watson@mines.edu for assistance with the supporting documentation (except police report).

Occasionally Mines will need to provide to another party evidence of insurance coverages for liability, property, workers’ compensation or other specifically identified insurance. Certificates of Insurance (COI) must be issued by our insurance broker (Flood and Peterson).  Issuance of certificates requires specific information regarding the event, function, provision of services, or use of facility for which the certificate is to be issued. All requests for a COI must be submitted in writing to the Office of Risk Management by a Mines faculty or staff member needing the certificate. All certificates are issued specifically for the intended purpose.
